Types implants for breast augmentation

Implants are largely divided into saline bags and silicone gels. However, saline bags are rarely used nowadays due to stability problems such as leakage of implant contents or replacement of implants. Silicone gel is almost impossible to break in the human body and is said to be the closest to a real breast in terms of elasticity and touch.

Round type implant – Although the demand for round type implant has been relatively low down since the release of the teardrop type implant, the round type implant is stillwidely using in for breast augmentation due to the needs of patients who want to emphasize the true shape of the breast or to make the upper breast look more voluminous.

Types of Incision

1. Armpit incision-most preferred method of incision since the scars are difficult to see, but may be more painful after surgery than other incisions. It is also a site where the incision heals quickly but the fibrillation of the scar is more likely to occur.

2. Areola incision – a good incision for sagging breasts but the sensation at the surgical site is likely to temporarily occur.

3. Inframmary incision – The incision is made about three to four centimeters along the underline of the breast. This is a good incision for surgery with large or teardrop type implants and healing period may be slightly slower than other incisions.

Insertion of the implant

Subpectoral – Insertion of implants under the muscles requires the amotion of part of the muscles, which can be painful and have a long recovery. However, the soft tissue covering the implants (muscles, mammary glands, fat, skin, etc.) is sufficient, so the outlines of the implants are rarely likely to appear, and you can expect a real breast texture. It also reduces the chance of sagging or capsular contraction ofbreast.

Submammary – less pain because it is a surgical method that does not strain the muscles. However, if your skin is thin, the outline of the implant will be more visible and more likely to develop fibrosis.

Getting ready for breast augmentation surgery

Take note of all instructions before the surgery during the consultation. This includes information about smoking, medications taken, tickets, what to bring during the stay, length of stay after discharged, and fasting before surgery.

Below are some things to keep in mind to prepare for surgery and what to do during recovery.

• Smokers should quit smoking for two weeks before and two weeks after surgery. Smoking increases the risk of side effects. When smoking, the chemicals in cigarettes act as free radicals and interfere with the postoperative recovery process. Long recovery periods can cause scars to form in large or strange shapes, cause inflammation due to poorly healed scars, and serious side effects that require removal of the implant. If you continue to smoke, your doctor may not proceed with the surgery.

• You should not drink alcohol one week before surgery.
• Do not consume any food or drink, including water, at least six hours before your surgery.

• Aspirin, anti-inflammatory medications that can cause bleeding, and vitamins, fish oils, and herbal supplements that can cause swelling should be stopped two weeks before surgery.

• This surgery cannot be performed while breastfeeding. Breast milk may contains bacteria that can contaminate the implants, cause inflammation, or cause capsular contraction. Surgery can be considered after at least three months. Stop takingcontraceptive pills or hormonesat least two weeks before surgery because it may cause production of breast milk.

Process of breast augmentation

1. An inframmary incision, areola incision, or an armpit incision will be made. The incision proceeds to an inconspicuous area, minimizing visible scarring. Consult with your doctor before surgery to determine the appropriate incision method.


2. Raises breast tissue and skin, exfoliates and creates space. (Subpectoral orsubmammary)

3. Place the implant in the space created.

4. Suture the incision.

5. Bandage and secure with medical tape.

After breast augmentation

Surgery is usually performed over an hour and a half through general anesthesia. After surgery, the patient recovers for about an hour under the observation of the anesthesiologist in the recovery room and then transferred to the room. You will be discharged after one or two days of hospitalization and the pain may persist for two to three days. After a week, follow check-up will be done by your doctor for abnormalities or remove the stitches. The doctor will then decide if you are fit to fly back home if no abnormalities are found. The swelling gradually subsides and disappears over 4-6 months.

• Avoid drinking and smoking for at least one month and get plenty of rest.

• Intense activities and sports should be avoided for at least one month.

• Avoid physical contact to your chest for at least 3-4 weeks.

• After removal of the stitches during the follow up check-up, follow your doctor’s instructions to wear a support bra without wires after most of the edema subsides one to two weeks after surgery.

• You can wear a bra with wire two to four months after surgery.

Q: How long does the implant last?

A: In general, the life span of implants is more than 10 years. This does not mean that you will need to replace the implant every 10 years.
