Types implants for breast augmentation

Implants are largely divided into saline bags and silicone gels. However, saline bags are rarely used nowadays due to stability problems such as leakage of implant contents or replacement of implants. Silicone gel is almost impossible to break in the human body and is said to be the closest to a real breast in terms of elasticity and touch.

Round type implant – Although the demand for round type implant has been relatively low down since the release of the teardrop type implant, the round type implant is stillwidely using in for breast augmentation due to the needs of patients who want to emphasize the true shape of the breast or to make the upper breast look more voluminous.

Teardrop type implant -If you want the most natural breast shape, the teardrop implant is recommended since it has a wide bottom and resembles the shape of water droplets or tears. If the breast tissue is poor and the skin is thin, the round type implant will make the upper breast look unnatural. In this case, if you select the teardrop type implant and perform the surgery, you can achieve a morevoluminous and natural looking breast.

Motiva implant – It is the most recent type of implant on the market, and it is reported that there is a lower occurrenceof side effects compared to the other types that can occur after breast augmentation. Side effects such as capsular contraction, fibrosis, double line, seroma phenomena has a lower occurrence in this type of breast implant. It is designed to have a so called “Blue Seal Technique” so that even in case of the implant getting damaged or broken the contents will not leak out ofthe implant and the chance of actually breaking the implant is very rare. You can check the details of the implants used through the “Q inside application”, and the “Bio sensors method” is applied to detect abnormalities by detecting body temperature. Compared with the other existing implants, the side effect rate is significantly lower and the texture is excellent. Another thing that sets this implant apart from the others is that the shape of the implant changes depending on posture. When in a standing position the breast will have a teardrop shapeand when laying down in will have a circular shape, which is very similar to a real breast.

Basically, the size of the patient’s thorax (chest) is the biggest factor in the selection of the implant, but there are various conditions such as the size and width of the original breast before surgery, skin thickness, elasticity, deflectionof the breast, distribution of breast tissue, height and overall body shape. After all the factors are considered, the most appropriate shape and size of the implant will be decided.

Am I suitable for breast augmentation?

Before proceeding with the surgery, think carefully about your expectations and possible risks and discuss with your doctor. The best candidates for breast augmentation are those who want to improve rather their figure rather than seek a perfect breast.

• Want to have a bigger overall breast size

• Your breast is small and your hips look relatively incongruous.

• Your breast is small resulting in you having limited choices in terms of clothing style and accessories.

• Unsatisfied with changes in the shape or size of the breast due to childbirth, weight loss, or aging

• Not satisfied with the flatness of your upper breast

• Your chest is asymmetrical or different size or shape

• Current breast implants have problems

• One or both side of the breast are not well developed or have an unnatural long shape

In addition, if your breast is drooping excessively, additional lifting may be required.

Most breast lifting surgerycan be performed concurrently with breast augmentation surgery, but some cases may require separate surgery after a sufficient recovery period, apart from breast augmentation surgery. This can be decided based on doctor’s opinion.

Types of Incision

1. Armpit incision-most preferred method of incision since the scars are difficult to see, but may be more painful after surgery than other incisions. It is also a site where the incision heals quickly but the fibrillation of the scar is more likely to occur.

2. Areola incision – a good incision for sagging breasts but the sensation at the surgical site is likely to temporarily occur.

3. Inframmary incision – The incision is made about three to four centimeters along the underline of the breast. This is a good incision for surgery with large or teardrop type implants and healing period may be slightly slower than other incisions.

Insertion of the implant

Subpectoral – Insertion of implants under the muscles requires the amotion of part of the muscles, which can be painful and have a long recovery. However, the soft tissue covering the implants (muscles, mammary glands, fat, skin, etc.) is sufficient, so the outlines of the implants are rarely likely to appear, and you can expect a real breast texture. It also reduces the chance of sagging or capsular contraction ofbreast.

Submammary – less pain because it is a surgical method that does not strain the muscles. However, if your skin is thin, the outline of the implant will be more visible and more likely to develop fibrosis.

Getting ready for breast augmentation surgery

Take note of all instructions before the surgery during the consultation. This includes information about smoking, medications taken, tickets, what to bring during the stay, length of stay after discharged, and fasting before surgery.

Below are some things to keep in mind to prepare for surgery and what to do during recovery.

• Smokers should quit smoking for two weeks before and two weeks after surgery. Smoking increases the risk of side effects. When smoking, the chemicals in cigarettes act as free radicals and interfere with the postoperative recovery process. Long recovery periods can cause scars to form in large or strange shapes, cause inflammation due to poorly healed scars, and serious side effects that require removal of the implant. If you continue to smoke, your doctor may not proceed with the surgery.

• You should not drink alcohol one week before surgery.
• Do not consume any food or drink, including water, at least six hours before your surgery.

• Aspirin, anti-inflammatory medications that can cause bleeding, and vitamins, fish oils, and herbal supplements that can cause swelling should be stopped two weeks before surgery.

• This surgery cannot be performed while breastfeeding. Breast milk may contains bacteria that can contaminate the implants, cause inflammation, or cause capsular contraction. Surgery can be considered after at least three months. Stop takingcontraceptive pills or hormonesat least two weeks before surgery because it may cause production of breast milk.

Process of breast augmentation

1. An inframmary incision, areola incision, or an armpit incision will be made. The incision proceeds to an inconspicuous area, minimizing visible scarring. Consult with your doctor before surgery to determine the appropriate incision method.

2. Raises breast tissue and skin, exfoliates and creates space. (Subpectoral orsubmammary)

3. Place the implant in the space created.

4. Suture the incision.

5. Bandage and secure with medical tape.

After breast augmentation

Surgery is usually performed over an hour and a half through general anesthesia. After surgery, the patient recovers for about an hour under the observation of the anesthesiologist in the recovery room and then transferred to the room. You will be discharged after one or two days of hospitalization and the pain may persist for two to three days. After a week, follow check-up will be done by your doctor for abnormalities or remove the stitches. The doctor will then decide if you are fit to fly back home if no abnormalities are found. The swelling gradually subsides and disappears over 4-6 months.

• Avoid drinking and smoking for at least one month and get plenty of rest.

• Intense activities and sports should be avoided for at least one month.

• Avoid physical contact to your chest for at least 3-4 weeks.

• After removal of the stitches during the follow up check-up, follow your doctor’s instructions to wear a support bra without wires after most of the edema subsides one to two weeks after surgery.

• You can wear a bra with wire two to four months after surgery.

Q & A

Q: Can I breastfeed after breast augmentation surgery?

A: Yes. Since surgery does not affect the feeding tube, you can feed normally.

Q: Can breast implants cause breast cancer?

A: No. Breast augmentation surgery or implants do not cause breast cancer. Implants are proven by US FDA approved and pose no risk to the human body.

Q: How long does the implant last?

A: In general, the life span of implants is more than 10 years. This does not mean that you will need to replace the implant every 10 years. If the patient does not feel any unusual signs or discomfort after having a breast implant, there is no need to permanently replace it. However, if 10 years have passed since you have had breast augmentation surgery, we recommend that you have a mammogram-like checkup to find out the status of your implant.

Q: I want to make my breasts very big. Can you do that?

A: The size of the breast implant is determined by the patient’s original physical condition. Forcing too large size can lead to unnatural breasts. The size of the implant will be explained in detail for each patient at the time of consultation with the attending physician.

Q: How long does swelling last after surgery?

A: After one month of surgery, large swellings subside and finally all swellings disappear over a total of three to six months.

Q: When can my husband touch my breast after surgery?

A: This is ok if the surgical incision is completely closed and pain is relieved. Sexual activities can be started after a month.
